Job Title:                     Administrative Assistant – Diabetes Team

Reports To:                 Manager of Operations

Primary Function or Purpose of Job or Position

Credit Valley Family Health Team (CVFHT) is an Academic Family Health Team affiliated with Trillium Health Partners and the University of Toronto. It is an engaging inter-professional primary care team dedicated to continuous learning and excellence within primary care with a demonstrated track record of impactful quality improvement activities. It is a not for profit organization that receives funding from the Ministry of Health and Long Term Care and provides primary health care services to over 12,000 patients in Mississauga. The CVFHT also has a mandate to provide French language primary care services to the Francophone community.

Diabetes Management Program, a regional service that provides education and self – management, supports the patients with diabetes and their families, within the Mississauga-Halton LHIN.  Medical Office Assistant supports the team by providing exceptional quality care to all patients and their families.

Medical Administrative Duties:

·         Retrieve incoming referrals from Mississauga-Halton Central Intake Diabetes Program through Novari from community physicians, specialists and CVFHT rostered patients. 

·         Registering all patients’ demographics accurately into Accuro.  

·         First point of telephone contact to schedule appointments as per triage directions from diabetes clinicians.  

·         Coordinating with Cliniconex for all patients’ appointment confirmations.  

·         Managing cancellations, no shows and follow up as per process. 

·         Communicating with physician when patient is discharged from program.  

·         Answering phone calls for all diabetes related inquiries.  

·         Communicates to team when demand is increasing and changes to the schedule required.  

·         Creating and managing diabetes team schedules templates in Accuro.  

·         Organizing, coordinating and managing diabetes group education and outreach activities.  

·         Coordinating diabetes monthly meetings, preparing agenda and taking minutes.  

·         Troubleshooting minor IT issues.

·         Ensures all diabetes program tasks and work is completed in a timely manner.  

·         Create, modify, run diabetes queries and assist with statistics for diabetes-related processes to report to Ontario Health.

·         Support Diabetes

·         Quality Improvement initiatives.  

·         Inventory management of supplies, maintenance of inventory and ordering supplies.

·         Other duties as assigned by the organization.

Qualifications / Skills:

Ability to speak French is an asset.
