The Credit Valley Family Health Team has partnered with The Credit Valley Hospital and GlaxoSmithKline to provide a PRIISME ™lung health program called “Breath for Life” for patients with asthma or chronic obstructive pulmonary disease (COPD)
The Breath for Life program provides lung function testing (spirometry), education on lung health and smoking cessation along with medication management and ongoing follow up for patients with mild to more severe forms of asthma or COPD.
The team includes: doctors, nurses, pharmacist, respiratory therapist and nurse practitioner.
You can be referred to the program by your primary care provider or you may refer yourself by calling 905 813-3850 and requesting an appointment to the Breath for Life program for asthma or COPD education/management.
